Preventing Zika disease with novel vector control approaches

The highest numbers of dengue cases in Latin America in the last few years have occurred in Brazil, Colombia, and Mexico. These countries have also faced outbreaks of chikungunya (2014-2015) and Zika (2015-2016). All three diseases are transmitted by the same mosquito vector, the aedes aegypti. The University of Ceará, Fundacion Santa Fe de Bogotá, and University of Yucatán have implemented vector control strategies to significantly reduce aedes aegypti.

This proposal aims to scale up participatory interventions targeting the surveillance, prevention, and control of aedes-transmitted diseases in three municipalities, namely Fortaleza (Brazil), Armenia (Colombia), and Merida (Mexico). Multidisciplinary teams will collect epidemiological and entomological indicators at the beginning, mid-term, and end of the project in a household sample of the intervention. The project will also assess the governance outcomes, and the costs and cost-effectiveness of the scaled intervention compared to the routine vector control program. A complementary feature will be the design and implementation of an innovative surveillance system that will engage the community to improve case detection.

Qualitative methods will be used to assess the acceptance of the intervention and to describe factors affecting the scaling up. State and municipal agencies, as well as NGOs, community-based organizations, and academic institutions will co-lead the scale-up of the interventions. Local and national resources have been committed to the implementation of the project. During the previous work of the principal research institutions in the three cities, strong links were developed with relevant strategic stakeholders that will facilitate implementation. The project is expected to make a significant contribution to the prevention of Zika, chikungunya and dengue disease through novel vector control and surveillance interventions that are potentially applicable in many other countries.

Fitting long-lasting insecticidal nets (LLIN) as screens on doors and windows has a significant impact on indoor adult Aedes aegypti, with entomological reductions measured in a previous study being significant for up to two years post-installation, even in the presence of pyrethroid-resistant Aedes populations. The study used bioassays (0, 6, and 12 months), which confirmed that LLIN residual activity decreased over time. However, the study demonstrates that the remaining chemical effect after field conditions still contributes to killing/repelling mosquitoes. LLIN screening from the neighborhood Juan Pablo II in Merida (Yucatan State, Mexico) were randomly selected. Merida is highly endemic for dengue and other Aedes-borne viruses.

There is strong and unquestionable epidemiological evidence of the protective efficacy of insecticide treated screens (ITS) against an arboviral disease of major relevance. Houses with ITS were 79-85% less infested with Aedes females than control houses during the peak of the zika epidemic, an effect that was significant up to a year. A similar significant trend was observed for blood-fed Ae. aegypti females (76-82%). Communities strongly accepted the intervention, due to its perceived mode of action, the prevalent risk for Aedes-borne diseases in the area, and the positive feedback from neighbours receiving ITS. The paper provides details of the study.

Houses are provided with long-lasting insecticidal nets (LLIN), permanently fixed with aluminium frames on doors and windows. Insecticide-treated house screening (ITS) proved to confer protection which showed 50% less chance of having indoor female Ae. aegypti in comparison with non-protected houses in urban areas of the Mexican cities of Acapulco and Merida. The brochure provides a brief encapsulation of the project and results. The study included the detection of dengue (DENV), chikungunya (CHIKV) and Zika (ZIKV) viruses. All of the project participants perceived effective mosquito reduction and recommend scaling-up of the intervention.

Results of the project show that pregnant women can be provided with free or low-cost integrated interventions with methods of known efficacy (topical repellent and insecticide-treated window screens) along with educative strategies, and that they are highly motivated to use these methods to enhance maternal-child health for Zika and other Aedes-borne diseases (ABD). This is a one-page assessment/synopsis of the integrated intervention.

Longitudinal Dengue (DENV) data generated patterns indicative of the resulting introduction and transmission patterns of chikungunya (CHIKV) and Zika virus (ZIKV), leading to important insights for the surveillance and targeted control of emerging Aedes-borne viruses. About 42% of the 40,028 DENV cases reported during 2008–2015 clustered in 27% of Merida (Mexico), and these clustering areas were where the first CHIKV and ZIKV cases were reported in 2015 and 2016. Findings from this article open a window to the consideration of spatially-targeted approaches for delivery of vector control interventions and surveillance.

Along with Aedes aegypti (L.), Aedes albopictus has been implicated as a secondary vector for dengue virus, chikungunya virus, and Zika virus in Latin America. The article covers an entomological survey in 2018, in a suburban area of Merida (the largest and capital city in Yucatan). This is the first report of Ae. albopictus in a suburban area of Merida City. A total of 259 specimens were collected. It is important to consider the ecology of Ae. albopictus alongside that of Ae. aegypti when developing vector/disease control programs.

Identifying urban hotspots of dengue, chikungunya, and zika transmission in Mexico to support risk stratification efforts : a spatial analysis

The study identifies persistent Aedes-borne disease hotspots in Mexican cities. In this spatial analysis, geocoded cases of dengue, chikungunya, and Zika from nine endemic cities were aggregated at the census-tract level. Findings show an overlap between hotspots of 61.7% for dengue and Zika and 53.3% for dengue and chikungunya. Dengue hotspots (2008–16) were significantly associated with those detected during 2017–20 in five of the nine cities. Heads of vector control confirmed hotspot areas as problem zones for arbovirus transmission. This study provides evidence of the overlap of Aedes-borne diseases within geographical hotspots and a methodological framework for the stratification of arbovirus transmission risk.

Houses protected with screens were 80% less infested with Aedes females and very importantly, had significantly less infected female Ae. aegypti during the peak of the Zika epidemic. Integration of house-screening and long-lasting insecticidal nets, known as insecticide treated screening (ITS), can provide simple, safe, and low-tech Aedes aegypti control. Cluster randomised controlled trials in two endemic localities for Ae. aegypti of south Mexico, show that ITS conferred both immediate and sustained impact on indoor-female Ae. aegypti infestations. Such encouraging results require further validation with studies quantifying more epidemiologically-related endpoints, including arbovirus infection in Ae. Aegypti.

Estimating absolute indoor density of Aedes aegypti using removal sampling

Exhaustive removal sampling represents a promising method for quantification of absolute indoor Aedes aegypti density, leading to improved entomological estimates of mosquito distribution. The study describes the use of sequential removal sampling to estimate absolute numbers of indoor resting Aedes in the city of Merida, Mexico. The study was performed in 200 houses based on recent occurrence of Aedes-borne viral illness in residents. The lack of a numerical association between relative and absolute density of adult Ae. aegypti represent a significant gap in vector surveillance. Merida is highly endemic for dengue and other Aedes-borne viruses.

Ecobiosocial approach and health promotion at school : building knowledge for community surveillance in Aedes aegypti control

The study aims to implement community surveillance actions for Aedes Aegypti vector control in school environments in light of health promotion and eco-bio-social approaches. This qualitative research addresses the assumptions of action research. Recommended as an innovative technology for vector control in Brazil, eco-bio-social approaches play an essential role in the sustainability of combating Aedes Aegypti and the diseases it transmits. The article follows the methodology, data collection and results of the project. Individual and community surveillance and social participation actions are essential to rethink vector control strategies for transmission of Dengue, Chikungunya and Zika diseases.

Effectiveness of an intervention for Aedes aegypti control scaled-up under an intersectoral approach in a Colombian city hyperendemic for dengue virus

The study designed a scaled-up community-led Aedes control intervention and assessed it for its capacity to reduce dengue in Girardot, Colombia. Reported dengue cases, and associated factors were analysed from Colombian disease surveillance systems data sets. The differences in dengue incidence among scaling-up phases (pre-implementation vs sustainability) and between treatment groups (intervention and control areas) were modelled. Evidence was found in favour of the intervention, although to maximise impact the scaling-up of the intervention should continue until it covers the remaining sectors. Aedes aegypti is the principal vector of dengue, chikungukya, Zika and yellow fever and is found on all continents except Antarctica.
